We believe that good health starts in the home. That's why we team up with health plans like yours to offer an In-Home Health Visit.

This visit is offered at no additional cost to you. It does not replace your regular healthcare visits, nor does it change your healthcare plan or benefits.

During the visit, an Easy Company clinician will do the following:

  • Check your vitals.

  • Review your medications.

  • Check for safety risks.

  • Discuss your medical history.

  • Answer any health-related questions you may have.

*When scheduling someone other than yourself and providing their contact information, you are acknowledging you have the authority to do so and to permit us to use the information in accordance with Easy Company's Privacy Policy.

Myself

Choose this option if you are the member scheduling a visit for yourself or your household.

 

Caregiver or healthcare provider

Select this option if you are a caregiver, relative, friend, or healthcare provider scheduling a visit on behalf of someone you care for.

 

Agent or Broker

Select this option if you are an agent or broker scheduling a visit on behalf of the member.
